Identifying and mitigating risks, managing adverse events, and preventing errors requires a proactive, systems-based approach focusing on root cause analysis, standardized communication, and a non-punitive culture. There are a number of established programs widely adopted to identify triggers and resultant harms analysis of Patient Safety quality, and costs
PROTeam Management provides direct support and participation in these programs with focused team support in collection analysis and dissemination within the methodology of each program(s).
IHI Global Trigger Tool (GTT) Reviews
The Institute for Healthcare Improvement (IHI) Global Trigger Tool for identifying and Measuring Adverse Events is a popular way to spot and count how often things go wrong (adverse events) in healthcare settings.
PROTeam Management acts as an independent reviewer, using the IHI GTT method to randomly select inpatient medical records monthly to identify adverse event triggers and their severity. It reports findings using combined control charts like Adverse Events per 1,000 Patient Days, Adverse Events per 100 Admissions, and the percentage of admissions with adverse events. This thorough analysis helps us identify and rectify errors to prevent recurrence.
Leapfrog Hospital Survey Support
The Leapfrog Group, a nonprofit founded by major companies, aims to improve patient safety, quality, and costs by collecting and sharing hospital performance data. It’s simple A, B, C, D, F letter grades (Leapfrog Hospital Safety Grade) provide clear information to patients, families, and buyers, helping them choose safer, better-value care and improve patient health by highlighting hospital performance on safety, infections, surgical errors, and more.
The survey spans 330 pages and examines 22 methods for assessing patient safety. Hospitals must answer numerous questions and submit supporting documents for each method. If you’re one of the 3,000 invited hospitals, you can choose to participate or not. Leapfrog will still assign a grade without your input.
Participating in the Leapfrog Hospital Survey is free for all hospitals, with no application or data submission fees. However, hospitals may incur voluntary costs related to participation and utilizing their results. For example, while the survey itself is free, collecting data and implementing safety practices can require substantial staff time.

Quality and Safety Review Systems (QSRS)
The Agency for Healthcare Research and Quality (AHRQ) Quality and Safety Review System (QSRS) provides a consistent, accurate, and comprehensive method for measuring patient harm and tracking adverse events in inpatient and emergency settings. It helps hospitals and regulators identify safety gaps, evaluate improvement initiatives, and report data aligned with AHRQ Common Formats.
PROTeam Management supports client participation collection and submission based on specific record requests, which must be promptly submitted to The CMS Clinical Data Abstraction Center (CDAC) for data extraction from selected inpatient discharges based on QSRS specifications.
